  • Shift times for both positions are days M-F with option to rotate weekends.
  • Hours are 0730-1830 to 1600-1700.
  • Employees have flexibility to pick their arrival times.
  • Candidates must have acute hospital Case Management experience.
  • NO ON CALL REQ
  • CCM PREFERRED BUT NOT REQ

    • CCM preferred but not required.
      • Schedule Information
      • Shift: start between 0730 & 0830, leave between 4pm & 5pm
      • Weekends: Voluntary only, not required
        • Unit Specific Information
        • Typical hiring profile: Travel RNs, local CNAs and sitters
        • Must have: Hospital case management experience of at least 6 months
        • Preferred or nice to have: Utilization Review background
        • List typical procedures performed on unit(s): Discharge planning

          • EMR - Meditech, Midas
          • Permanent Positions on Unit Available? (Y/N) - Y
